Lifestyle outpatient clinic

After visiting the CVA follow-up outpatient clinic, all results will be discussed in a team of specialists and specialized nurses. Recommendations for a treatment plan will be given based on these results, for example the recommendation to lose weight, quit smoking or change medication. These results will be discussed with you and we will discuss with you whether you will implement these changes with help from us at the outpatient clinic or from the family physician/practice support staff.

Often there are regional agreements in place. You can discuss this with your treating physician. The follow-up care is often not tailored specifically to people who have suffered a stroke at a young age.

At the CVA follow-up care outpatient clinic, you will meet with the specialized nurse or practice support worker. You can discuss questions or problems that have risen after your discharge. read more

At the CVA follow-up care outpatient clinic, you will meet with the specialized nurse or practice support worker. You can discuss questions or problems resulting from your cerebral infarction, cerebral hemorrhage or TIA, such as problems with memory, attention, concentration and fatigue, side effects of medication or new symptoms. As there is a clear link between lifestyle and the occurrence of cardiovascular diseases, you will be asked to complete a digital questionnaire prior to your first visit to the outpatient clinic. If you do not have a computer, then the nurse or practice support worker will complete the questionnaire with you during your first visit to the outpatient clinic.
