At the CVA follow-up care outpatient clinic, you will meet with the specialized nurse or practice support worker. You can discuss questions or problems resulting from your cerebral infarction, cerebral hemorrhage or TIA, such as problems with memory, attention, concentration and fatigue, side effects of medication or new symptoms. As there is a clear link between lifestyle and the occurrence of cardiovascular diseases, you will be asked to complete a digital questionnaire prior to your first visit to the outpatient clinic. If you do not have a computer, then the nurse or practice support worker will complete the questionnaire with you during your first visit to the outpatient clinic.

These results will be discussed with you and we will discuss with you whether you will implement these changes with help from us at the outpatient clinic or from the family physician/practice support staff.
